Male Rhinoplasty Revision!
A case study of a male patient in his 20s who underwent rhinoplasty revision to correct a deviated and drooping nasal tip using donated rib cartilage.
* Basic Information
# Male in his 20s
# Previously underwent rhinoplasty using septal cartilage, but the nose appeared deviated to one side. Over time, the nasal tip drooped, causing the bridge to look convex or bumpy.
* Consultation
# The patient visited due to two main issues following his initial surgery with septal cartilage: 1. Progressive deviation of the nasal tip and 2. A drooping tip that made the bridge appear protruded.
# Since the septal cartilage had already been used, we planned to proceed with the surgery using donated rib cartilage.
* Surgical Plan
# Nasal Tip Surgery (Correction of deviation & drooping tip)
# Nasal Bridge Surgery (Implant replacement)
* Intraoperative Findings
# Severe scar tissue was present; it was thoroughly removed, and the area was irrigated with an anti-scarring agent.
# The septum was deviated, and the previous septal extension graft was weakly fixed to that area. This was the primary cause of the deviated nasal tip.
# The septum was reconstructed. Using the existing septal cartilage combined with new donated rib cartilage, a sturdy septal pillar was newly created.
# Additional cartilage was grafted onto this strong pillar to increase the height of the nasal tip.
# When performing revision surgery, it is crucial to establish a perfect plan and prepare thoroughly beforehand. One must consider various variables in advance to respond flexibly to unexpected situations that may arise during surgery!
