A case study of a 20s female patient who achieved a more feminine and refined look through comprehensive rhinoplasty addressing a hump, deviation, and bulbous tip.

* Consultation
1. The patient wanted to improve her flat and wide-looking face caused by a wide and low nasal bridge. She desired a more feminine line rather than a masculine feel.
2. She was also stressed about looking older than her actual age due to the long, drooping nasal tip.
3. We planned to create a shorter nasal tip compared to her current state, while ensuring it did not look too upturned. Bulbous nose correction was also planned to create a slimmer appearance.
4. The pre-operative CT scan showed a severely deviated septum, which suggested limitations in using it as the sole material for nasal tip extension. Therefore, we planned to use rib cartilage as well to construct the tip.

1. In the post-operative results, a natural yet sharp nasal tip was achieved without excessive nostril exposure, exactly as the patient requested.
2. Also, before the surgery, the columella appeared retracted due to the drooping tip. By protruding the columella, the nasolabial angle was improved, making the line from the nose to the philtrum look much more sophisticated and beautiful.
3. As the wide bridge became narrower and higher, the gaze became more centered. This added three-dimensional depth to the previously flat face, creating an effect where the face looks smaller, which the patient was very satisfied with.

Yes, it is possible. If a CT scan before surgery reveals severe septal deviation, making the septum insufficient as an extension material, additional autologous cartilage such as rib cartilage can be used to create a strong and natural-looking nasal tip.
How are a hooked nose and a wide nasal bridge corrected?
The hooked part is trimmed, and osteotomy is performed to reduce the width of the nasal bridge. After smoothly refining the protruding hump, the wide nasal bones are fractured and brought together, and if necessary, a silicone implant is placed to complete a three-dimensional and feminine nasal bridge line.
If I undergo surgery for a long, drooping nasal tip, will it become an upturned nose?
No, that is not the case. Through pre-surgical consultation, a precise plan is made to raise the nasal tip while ensuring the nostrils are not excessively visible. By appropriately extending the columella and enhancing the nasolabial angle, a sophisticated nasal tip is achieved without appearing upturned.
What effects can I expect from bulbous nose correction?
You can expect the nasal tip to become slender and for your entire face to gain more dimension. By gathering the blunt nasal tip and narrowing the wide nasal bridge, attention is drawn to the center of the face, making a flat face appear smaller and transforming it into a more refined impression.
