This patient is a woman in her 30s who had repeatedly received filler injections in her forehead and bridge of the nose over a long period.
Over time, the filler had spread and clumped, causing the overall facial contour to become uneven and bumpy.
In particular, the forehead line was not smooth, and the filler on the bridge of the nose had spread wide, giving the center of her face a heavy and stifled appearance.
The first step we took before surgery was the ‘process of sufficiently dissolving and clearing the existing filler.’
If surgery is performed while a significant amount of filler remains, it is difficult to accurately judge the actual structure of the bone and soft tissue, which can lead to unnatural results.
The Cause of a Heavy and Harsh Impression
Looking at the pre-operative photos, the eyebrow position was generally low, and she was using a lot of forehead muscle strength when opening her eyes.
In such cases, it is common for:
- The eyes to look heavy and stifled
- The person to look sleepy
- The impression to appear sensitive or harsh
Specifically, when the eyebrows are pressed downward, the distance between the eyes and eyebrows narrows, making the mid-face look heavy and the overall facial atmosphere feel congested.
Additionally, the three-dimensional volume of the forehead and nose bridge had collapsed due to the spread filler, blurring the central lines of the face.
The Combination of Endoscopic Forehead Lift + Non-Incisional Ptosis Correction
1. Endoscopic Forehead Lift

In this surgery:

- The tip was stably supported using autologous rib cartilage
Rather than just increasing height, we designed it to naturally establish the center in proportion to the entire face.
