A Three-Dimensional Side Profile Completed with Rhinoplasty & Forehead Fat Grafting
Hello.
I am Dr. Young-moon Yoo, Director of VIBE Plastic Surgery.
This patient was a woman in her 20s whose primary concerns were a low, wide-looking nasal bridge and a bulbous nose tip.
Additionally, while she had a relatively wide forehead, she was concerned that her overall face appeared flat. Therefore, this surgery was planned not just to raise the nose, but to create overall dimension and balance for the entire face.
Surgical Plan
1. Rhinoplasty
- Bulbous nose & nasolabial angle correction
- Improvement of the spread-out nasal line
- Use of donated rib cartilage
The patient had a low nasal bridge, but it was more important to refine the nose tip rather than simply increasing the height. Using rib cartilage to stably support the tip, we reduced the bulbous appearance and adjusted the nasolabial angle to create a more sophisticated impression.
2. Forehead Fat Grafting
Although the forehead was wide, it lacked volume in the center, making it appear relatively flat. Through forehead fat grafting, we naturally reinforced the volume of the front forehead to create a smooth curve. Forehead volume plays a more significant role than one might think in creating a feminine impression.
Why is it beneficial to combine Rhinoplasty and Forehead Fat Grafting?!
Many people focus only on the nose when considering rhinoplasty. However, a person’s side profile is not determined by the nose alone. In reality, the harmony of the entire line connecting the:
Forehead → Glabella → Nasal Bridge → Nose Tip → Chin
is much more important. If the nose is raised while the forehead remains sunken, the nose can appear disproportionately protruded. Conversely, when appropriate volume is formed in the forehead, a much more sophisticated and three-dimensional impression can be achieved even without an excessively high nose.
In particular, you might consider both procedures together in the following cases:
✔ If the forehead is flat or sunken
Even if the nose is raised, the desired three-dimensional effect may not be achieved.
✔ If the side profile (E-line, Profile line) is a priority
The curve leading from the forehead to the nose tip is completed much more naturally.

Post-Surgical Changes
After the surgery, the bulbous tip was improved, resulting in a more defined nose tip. Additionally, the volume added by the forehead fat grafting made the line from the forehead to the nose tip much smoother and more natural. From the front, the face appears more three-dimensional, and from the side, a sleek profile line has been formed. Most importantly, rather than looking like she ‘had her nose done,’ the biggest feature is that her entire face has transformed into a sophisticated and balanced impression.
