When I consult with patients considering plastic surgery, I often hear the question, "Can I just get my nose raised?"

However, the three-dimensional look of the face isn't determined by the nose alone.

Especially in profile view, the line created between the forehead and the nose often defines the overall impression.

Today's patient also felt that her low, flat-looking nose was her biggest concern.

But upon closer examination, she also lacked volume in the central forehead and in the under-eye + front cheek areas,

and improving those together would deliver a much better overall result.


<Concerns Before Surgery>

The patient is a woman in her 20s with a naturally pretty face shape overall, but she was concerned about:

✔ A low, flat-looking nose

✔ Lack of definition at the nasal tip

✔ Insufficient volume in the central forehead

✔ An unsatisfying line flowing from the forehead down to the nose

From the front, these features weren't very noticeable, but in profile, her forehead appeared slightly flat,

<Surgical Plan>

1. Fat Grafting (forehead + temples, front cheeks + under-eyes, chin tip)

The patient's overall forehead volume wasn't bad.

However, the area running from above the eyebrows to the central forehead was somewhat flat, leaving the line connecting to the nose underwhelming.

So rather than making the forehead excessively rounded,

we performed fat grafting in a way that naturally filled only the deficient areas.

The goal of fat grafting is not simply to enlarge the forehead,

but to enhance the three-dimensional look of the entire face and create a natural connection with the nose.

2. Rib Cartilage Rhinoplasty

Her biggest concern was the low, flat appearance of her nose.

To stably raise the nasal tip and ensure long-term durability, we used donor rib cartilage.

Using donor rib cartilage, we built a strong support structure,

and carefully adjusted the direction and height of the nasal tip to complete a more refined line.

We also refined the flat, spread-out appearance of the nose, creating a sharper, more defined image even from the front.

Through consultations, I've found that many patients actually have this type of facial structure.

① When the nose is low AND the forehead is flat

In this case, raising only the nose can make the connection between the forehead and nose look unnatural.

Adding proper volume to the forehead can create a much more refined three-dimensional look without over-elevating the nose.

② When the profile looks flat

Sometimes the face looks fine from the front, but appears flat in profile photos.
