# History of rhinoplasty several years ago (+)

# Over time, the columella has gradually retracted and the nasal tip has drooped. Her main complaint is that her nose looks too bulbous in photos, making it the only prominent feature on her face.

* Consultation

# The patient visited our clinic because she became increasingly dissatisfied with the shape of her nose years after her initial surgery.

# She mentioned that the retracted columella and blunt nasal tip were her biggest concerns. While a bulbous nose was traditionally called a ‘bok-ko’ (lucky nose) in Korea, many people now seek improvement because it can create a heavy or blunt impression.

# Additionally, the significantly retracted columella gave her profile a somewhat ‘stuffy’ or closed-off look. We planned to create an ideal nasolabial angle through columellar lengthening (columellar pull-out).

# Since she also felt stressed about her nostrils flaring whenever she smiled, we decided to perform a non-incisional alar reduction simultaneously.

# By correcting the bulbous tip, we achieved a slimmer and more refined nose.

# Before surgery, the nasal tip was drooping and the columella appeared retracted. By using the nasal septum and donated costal cartilage to pull the columella out, the nasolabial angle was restored. The line connecting the nose to the philtrum now looks much more sophisticated and beautiful.
