The patient I am introducing today is a woman in her 50s who traveled all the way from Japan.

👀 Pre-operative Condition

This patient was showing typical signs of middle-aged facial aging.

• A narrow, uneven forehead with sagging that made the eyes look heavy and closed off.
• Sagging eyelid skin covering her original natural double eyelids.
• A tired impression caused by protruding under-eye fat and sagging in the midface (cheek) area.

Due to these complex changes, her overall facial expression appeared heavy and dark. The patient’s goal was: ‘I want to look younger naturally, without it looking obvious.’

🩺 Surgical Plan

1️⃣ Endoscopic Forehead Lift + Subbrow Lift

No double eyelid surgery was performed.

Instead, through an endoscopic forehead lift and subbrow lift, the sagging skin was lifted upward, allowing her original natural double eyelids to be revealed again.

By using a minimal incision method with an endoscope, the burden of scarring is low and recovery is fast, resulting in clear and refreshed eyes without an artificial feel.

An additional subbrow lift, which excises skin directly below the eyebrows, was performed to tidy up the heavy upper eyelids, making the eye area look light and vibrant.

2️⃣ Lower Blepharoplasty + Midface Lift

The bulging fat under the eyes was organized, and fat was repositioned into the sunken areas. Thanks to this, the under-eye area became smooth and shadows disappeared.

A midface lift was performed simultaneously to pull up the sagging in the cheeks and nasolabial folds, restoring elasticity and volume to the center of the face.

The design focused on creating a natural lifting line without excessively pulling the skin.

🌸 Post-operative Changes

After the surgery, the eyes and the overall facial impression became surprisingly soft and bright.

1️⃣ The under-eye area became smooth, and the tired look disappeared.

By organizing and repositioning unnecessary fat through lower blepharoplasty, the bulges and wrinkles under the eyes vanished, and the skin texture became smooth. Fine lines and shadows were reduced, improving the eye area to a bright and clear impression.

2️⃣ Midface volume was lifted upward, completing a three-dimensional youthful face.

As the sagging cheek fat returned to its proper place, the area below the cheekbones and the nasolabial folds were naturally filled, resulting in a dimensional, youthful face. The corners of the mouth rose naturally, changing the impression to a soft, smiling look. It wasn’t just about removing wrinkles; the structure of the center of the face was lifted upward, greatly improving overall harmony.

3️⃣ Formation of a natural lifting line from the forehead to the eyes and cheeks.

As the upper face opened up with the forehead lift and the lower face was lifted with the midface lift, a unified lifting effect appeared as if the entire face had naturally moved upward. We achieved a youthful impression with soft and natural expressions without any artificial pulling.
