5th Rhinoplasty Revision and Endoscopic Forehead Lift!
A case study of a female patient in her 30s undergoing her 5th rhinoplasty revision and an endoscopic forehead lift to improve a harsh impression and drooping nasal tip.

* Basic Information
# Female in her 30s
# Medical History
1. Double eyelid surgery & Lower blepharoplasty (fat repositioning)
2. Total of 4 previous rhinoplasties (the last one used autologous rib cartilage)
# Main Concerns: ‘The space between my eyebrows and eyes is narrow, making me look slightly angry. I also want to raise my nasal tip a bit more!’
* Consultation
# The patient visited because she was dissatisfied with the narrow, bulging space between her eyebrows and eyes following her double eyelid surgery, which gave her a harsh impression. The forehead and eyes cannot be considered separately; harmony between these two areas is essential for an ideal eye shape. Therefore, we planned an endoscopic forehead lift to ensure the eyebrows are positioned in an ideal shape and location.
# Despite four previous rhinoplasties, she was still unhappy with the nasal tip, which appeared low and drooping. Although the skin elasticity was limited, we planned to improve the ‘long nose’ look by raising the tip as much as possible.
* Surgical Plan
# Endoscopic Forehead Lift
# 5th Rhinoplasty Revision

* Post-operative Progress
1. By improving the bulging of the eyelids and the shape of the eyebrows, her previously harsh impression has softened, making her look much brighter!
2. When undergoing double eyelid surgery, if it is expected that the eyebrows will drop due to eye-opening habits or muscle strength, an endoscopic forehead lift can be of great help.
3. Since the forehead muscles are released, the forehead volume is naturally restored, resulting in a beautiful forehead as an added bonus!
4. Having already undergone four rhinoplasties, the patient had very little skin laxity. Since the existing columellar strut made of autologous rib cartilage was relatively stable, we preserved it as much as possible. By using additional ear cartilage to correct the nasolabial angle and further elevate the nasal tip, we achieved a very satisfying result!

Frequently Asked Questions
When is a forehead lift necessary?
A forehead lift is helpful when the distance between the eyebrows and eyes is narrow after double eyelid surgery, making the impression look fierce, or when the eyelids appear puffy. It is also effective when it is expected that the eyebrows will descend, considering the habit and strength of opening the eyes.
Can existing cartilage be reused during revision rhinoplasty?
Yes, it is possible. Depending on the patient's condition, if the previously used autologous costal cartilage columella is not weak, it can be preserved as much as possible and utilized for revision surgery. Additionally, ear cartilage can be used to correct the shape of the nasal tip.
Does a forehead lift also improve forehead volume?
Yes, a forehead lift can naturally enhance forehead volume as a secondary effect because it relaxes the forehead muscles. This can create a more beautiful forehead line.
Is revision rhinoplasty possible even after multiple nose surgeries?
Yes, it is possible. The patient in this case also underwent a fifth revision surgery after four previous nose surgeries. Even if there isn't much skin laxity, improvements can be attempted by preserving existing cartilage and adding ear cartilage to raise the nasal tip.
Why is the harmony between the forehead and eyes important?
The forehead and eyes are inseparable areas. The harmony between these two areas is essential for achieving an ideal eye shape and overall impression. The ideal position and shape of the eyebrows significantly influence one's impression.
