Lifting Revision Case for 60s (Endoscopic Forehead Lift / Lower Blepharoplasty & Midface Lift / Face & Neck Lift All-in-One!) - VIBE Plastic Surgery Dr. Yoo Young-moon
A comprehensive revision lifting case for a patient in her 60s, addressing forehead, eyes, midface, and neck sagging 10 years after her initial surgery.

Hello,
I am Dr. Yoo Young-moon, Director of VIBE Plastic Surgery.
Today, I would like to introduce a revision lifting case for a female patient in her 60s who had undergone a facelift and lower blepharoplasty about 10 years ago.
As time passed, she experienced sagging across her forehead, eyelids, midface, and jawline, leading her to seek a more natural yet definitive improvement.
For this surgery, we precisely categorized the aging areas and applied the most suitable methods comprehensively and three-dimensionally to each part.
🔎 Patient’s Primary Concerns
The forehead and eyebrows have descended, making the eyes look heavy and closed-in.
Recurrence of lower eyelid sagging and fat bulging.
Cheek sagging, deepening nasolabial folds, and jowls.
Sagging skin on the jawline and neck.
Progression of sagging over time following a previous facelift.
**📌 1. Endoscopic Forehead Lift
— When eyes look heavy and are accompanied by eyelid sagging**
▶ Why is this surgery necessary?
For those whose eyebrows have sagged downward, making the eye area look narrow.
For those with slight eyelid sagging who do not necessarily want double eyelid surgery or upper blepharoplasty.
For those with deep forehead and frown lines.
When the overall impression looks tired due to a drooping eye shape.
▶ Who is it recommended for?
Patients in their 50s–60s where the cause of eyelid sagging is brow ptosis.
Those who have had previous upper blepharoplasty but still look heavy-eyed.
Those who want a ‘natural, younger-looking effect’ without obvious signs of surgery.
▶ Reason for application in this case?
The patient’s eyebrows had descended significantly, narrowing the distance between the eyes and creating a heavy impression on the upper face.
Using an endoscope through small incisions, we achieved minimal scarring while extensively dissecting the forehead and glabella muscles to clearly improve the openness around the eyes.
**📌 2. Revision Lower Blepharoplasty + Midface Lift
— Simultaneously resolving lower eyelid sagging and midface (below cheekbones) hollowness/sagging**
▶ Indications for Revision Lower Blepharoplasty?
When sagging or bulging has recurred after a previous lower blepharoplasty.
Bulging fat under the ‘aegyo-sal’ (love band) area where fat has pushed forward.
Fine wrinkles and loss of elasticity under the eyes.
When the lower eyelid looks pulled or the shape appears unnatural.
▶ Indications for Midface Lift?
Sagging of the cheeks below the cheekbones (jowls) moving downward.
Deepening of the nasolabial folds.
When the center of the face looks elongated and aged.
Central sagging that cannot be resolved with fat grafting alone.
▶ Reason for application in this case?
The patient had both under-eye sagging and midface cheek sagging, making it a case that required a ‘total under-eye + midface lift’ rather than simple fat removal.
After repositioning and rearranging the fat through lower blepharoplasty, the midface was lifted to complete a more natural and youthful look.
**📌 3. Revision Face & Neck Lift
— The key to improving the jawline, jowls, and neck sagging**
▶ When is this surgery necessary?
When the jawline has collapsed and the face appears square.
When the neck skin is sagging and ‘banding’ is visible.
When sagging has returned 7–10 years after a previous facelift.
When non-surgical methods like thread lifts or lasers provide insufficient results.
▶ Who should get it?
Generally those in their 50s–70s.
When the cause of sagging originates in the SMAS (Superficial Muscular Aponeurotic System) layer.
Those who want a fundamental lift that cannot be achieved with threads or lasers.
▶ Reason for application in this case?
The patient showed sagging in the jawline and neck area as a significant amount of time had passed since her facelift 10 years ago.
This time, we aimed for a definitive and long-lasting lifting result by accurately dissecting the SMAS layer and adjusting the platysma (neck muscle).

✨ Post-Surgery Improvement Points
The eye area looks refreshed and the overall impression is brighter.
Improvement in under-eye bulging and fine wrinkles.
Natural volume recovery in the cheeks and nasolabial areas.
The jawline and neck line have become smooth and distinct.
An overall impression that looks 10 years younger.
