The surgical case I would like to introduce today is the ‘Abdominal Liposuction and Abdominoplasty (Tummy Tuck)’ of a woman in her 40s with large tattoos on her abdomen.
As a patient with an obese body type, she suffered from severe loss of elasticity and sagging skin. Furthermore, because a large tattoo spanned across her entire abdomen, this case required a high degree of precision in design and incision placement.
Surgical Background – Functional Concerns Beyond Simple Aesthetics
Due to years of weight gain, this patient experienced severe sagging of the abdominal skin, which led to difficulties in hygiene management, decreased self-esteem, and general discomfort in daily life.
Additionally, since she already had a large tattoo across her abdomen, the key point of the surgery was not just to excise skin or smooth out wrinkles, but to align the design so that the tattoo would connect naturally after the procedure.
Surgical Plan – A Unique Approach to Design
Abdominoplasty is typically performed by excising excess skin, pulling the lower portion down, and suturing it to the upper portion. However, when tattoos are present, an incorrect incision can cause the tattoo to appear torn or misaligned, significantly lowering patient satisfaction.

As a result, we were able to achieve a smooth abdominal contour while maintaining the natural appearance of the tattoo without tearing or misalignment.
