Incisional Ptosis Correction (Revision) & Lower Blepharoplasty (Fat Repositioning)
A case study of a 20s female patient undergoing revision incisional ptosis correction and lower eye fat repositioning to fix asymmetry and a tired appearance.

* Basic Information
# Female in her 20s
# Had incisional double eyelid surgery & epicanthoplasty 10 years ago
# Primary complaints and goals: The eyelid lines have loosened and become asymmetric. I want defined lines. My under-eyes look hollow and tired.
* Consultation
# The double eyelid lines have become loose and asymmetric. The patient is also concerned about multiple folds caused by hollow upper eyelids. She desires a more defined line through revision surgery.
# We decided to proceed with an incisional revision to remove scar tissue from the previous surgery and create a secure, defined line that will not loosen. Simultaneously, we planned to improve the hollow upper eyelids through fat repositioning if any upper eyelid fat remained (if it hadn’t been entirely removed during the previous surgery).
# For the under-eye area, we planned to improve the hollow appearance through fat repositioning.

* Post-operative Progress
# During the surgery, it was discovered that the upper eyelid fat had been excessively removed previously. There was no fat left to reposition. Therefore, during the lower eye fat repositioning, a portion of that fat was utilized for micro-fat grafting into the upper eyelids.
# Revision surgery using the incisional method has the advantage of allowing the removal of old scar tissue and ensuring a secure fixation.
# Bulging under-eye fat can create a swollen and tired impression. Lower eye fat repositioning can be performed relatively easily through the conjunctiva inside the eye without visible scarring, making it an excellent option for those with minimal skin sagging.
