Revision Rhinoplasty Using Autologous Costal Cartilage (+ Functional Rhinoplasty)
A case study of a 20s female patient who underwent revision rhinoplasty using autologous costal cartilage to correct a deviated septum and improve nasal tip projection.

* Basic Information
# Female in her 20s
# History of double jaw surgery and rhinoplasty several years ago (+)
* Consultation
# She had undergone nasal tip surgery using septal and donor rib cartilage several years ago, but her main complaints were an uneven nasal bridge and the nasal tip not being as high as she desired.
# Additionally, as time passed, the columella gradually became crooked, and one nostril became blocked, causing breathing discomfort.
# Due to the severe degree of deviation observed on the pre-operative CT scan, we planned to perform the correction using ‘autologous costal cartilage’.

* Intraoperative Findings
# Scar tissue was severe, so it was thoroughly removed, and the area was irrigated using an anti-scarring agent.
# The septum was severely deviated, and the previous septal extension graft was weakly fixed to that area.
# The septum was reconstructed using autologous costal cartilage. By improving the deviated septum, the symptoms of nasal congestion were also corrected.


* Postoperative Findings
# You can see that the nasal tip has become more defined and lifted compared to before the surgery.
# Autologous costal cartilage is typically harvested from the 7th or 8th rib, and there is slight pain for about 2-3 days after harvesting. After about a week, recovery is sufficient to return to daily activities without issues.
# When performing revision surgery, it is crucial to establish a perfect plan and prepare thoroughly beforehand. One must consider various variables in advance to respond flexibly to unexpected situations that may arise during surgery!
