Today, we would like to introduce a case of a patient in their 50s who was deeply concerned about a sagging jawline caused by double chin fat and tissue laxity.
Many people believe a double chin is simply an issue of excess fat.
However, with age, sagging of the skin and soft tissues occurs simultaneously,
making it difficult to achieve a satisfactory jawline merely by removing fat.
This patient's situation was not just a simple issue of double chin fat.
In addition to fat accumulation in the submental area,
there was significant skin and tissue laxity, and the structural support beneath the chin had weakened,
leading to a collapsed overall jawline.
Therefore, rather than performing simple liposuction alone,
we performed:
① Liposound double chin liposuction
② Submental muscle tying (platysmaplasty)
Double chin in your 50s: Why does the jawline become a concern?
In your 20s and 30s, a double chin is relatively often caused by fat accumulation alone.
However, from your 40s and 50s onwards, the story changes.
As we age, skin elasticity decreases,
and the soft tissues located under the chin begin to gradually sag downward.
When submental fat is added to this, the boundary between the chin and the neck becomes increasingly blurred.
As these changes progress,
the jawline becomes blunt,
the boundary between the chin and neck collapses,
the double chin appears more prominent,
the face looks larger and heavier than it actually is,
and from a profile view, the neck and chin appear to merge as one.
This patient was also
facing a situation where excess fat and sagging were occurring simultaneously,
so we determined that surgery to remove fat alone would not be sufficient.
Preoperative Analysis | Why wasn't liposuction alone enough?
Double chin liposuction is a very effective method for removing unwanted fat under the chin.
However, removing fat does not resolve every double chin concern.
In particular, when sagging of the skin and underlying tissues is severe,
the sagged tissues themselves remain even after fat removal, meaning the jawline may not become sharp enough.
In this patient's case,
"double chin fat + soft tissue sagging"
coexisted.
Therefore, the goal of the surgery was not simply to "remove a large amount of fat."
It was essential to remove the fat and reconstruct the sagging submental structure together,
creating a clearer boundary between the chin and the neck.
For this very reason, in this case,
we performed "Liposound liposuction" combined with "submental muscle tying."
What is Submental Muscle Tying (Platysmaplasty)?
Submental muscle tying is a surgical procedure that addresses severe laxity under the chin by
realigning stretched or separated submental muscles and soft tissues,
gathering them toward the center, and securing them firmly.
Beneath our chin lie several soft tissues and muscles that define the shape of the neck and jawline.
As we age, when these tissues sag downward or separate horizontally,
the area beneath the chin can look blunt and droopy, even without a large volume of fat.
In such cases, removing fat alone might reduce volume,
but it remains difficult to adequately improve the sagging structure itself.
Submental muscle tying acts to tightly reorganize these sagged tissues
and restore structural support beneath the chin.
In simple terms,
if liposuction is a surgery that "reduces unnecessary volume,"
