REAL Story 1: Eye, Nose, and Facial Line Transformation
A comprehensive transformation story of a patient in her 20s who underwent revision eye and nose surgery along with facial contouring to achieve a defined, slim look.

* Basic Information
# Female in her 20s
# Medical History: Incisional double eyelid surgery (twice), epicanthoplasty, lateral canthoplasty, alar reduction
* Diagnosis of Current Condition
# Eyes
- Eye-opening strength is not bad, but the low position of the eyebrows creates a heavy, pressing look on the eyes.
- The narrow space between the eyebrows and eyes results in a bulging and stuffy appearance.
- Scars from the two previous eyelid surgeries are prominent, and the eyelids feel heavy and puffy.
# Nose
- Low nasal bridge and drooping nose tip.
- The bridge line appears uneven due to a hump.
- The cartilage is very thin and lacks structural strength.
# Face Shape
- The face looks round and heavy due to fat in the cheeks and double chin area.
* Desired Changes
# Eyes
- Wants to improve the loosened lines and multiple folds to achieve a clear, large ‘out-line’ (parallel) double eyelid.
- Wants to reduce the puffiness of the eyelids for a more refreshed and defined look.
- Previously had lateral canthoplasty but saw no effect; wants to open up the outer corners more effectively.
# Nose
- Wants to smooth out the uneven bridge and achieve a sleek ‘straight-semi-basal’ (straight-curved) line.
- Wants the low, wide-looking bridge to appear more defined and elongated.
# Facial Line
- Wants a smoother V-line.
- Desires a sleek profile and jawline.
* Surgical Plan
# Eyes
- Endoscopic Forehead Lift
- Incisional Double Eyelid Surgery (3rd revision)
- Mango Canthoplasty (2nd revision)
# Nose
- Rhinoplasty (Hump reduction, silicone implant)
- Tip Plasty (Donated rib cartilage: Septal extension graft + Ear cartilage: Layered graft)
# Face Shape
- Ultrasonic Liposuction (Double chin, jawline)
- Buccal Fat Removal

* Post-operative Progress
# Eyes
- A bold out-line was successfully created, yet the eyelids do not look puffy at all. A light and refreshed eye shape was achieved.
- Through incisional revision surgery, all previous scars were removed, and we focused on creating a natural look as if it were her first surgery.
# Nose
- As expected during surgery, the septal cartilage was extremely thin and weak. We also discovered a severe deviation (signs of a past fracture) in the middle of the septum.
- While preserving the septum as much as possible, the tip was reconstructed using donated rib cartilage and ear cartilage. This resulted in a natural yet sharp, straight-curved tip line.
- The bridge was smoothed and heightened, drawing attention to the center of the face. This added three-dimensional depth to a previously flat face, making it appear smaller and more balanced, which the patient was very satisfied with.
# Face Shape
- The V-line was completed through ultrasonic liposuction (double chin, jawline) and buccal fat removal.
- The overall look appears much lighter and more refined, doesn’t it?
