Third Bulbous Nose Revision Surgery Using Autologous Costal Cartilage!!
A case study of a 30-year-old female undergoing her third rhinoplasty for a persistent bulbous nose and ptosis correction using autologous rib cartilage and incisional eye surgery.

* Basic Information
# Female in her 30s
# Medical History: 2 previous rhinoplasties (bulbous nose correction), Alar reduction (+)
# Current Concerns
1. Even after two surgeries, I am still bothered by my persistent bulbous nose :(
2. I have natural double eyelids, but they are too high, and I often hear that I look sleepy~
* Consultation
1. Nose
# The patient visited because her nose remained bulbous despite two previous surgeries.
# The nasal tip appeared even more blunt as it was filled with post-operative scar tissue.
# Since the septal and ear cartilages had already been used, I determined that a stronger and more substantial material was needed to overcome the thick skin. Therefore, a revision rhinoplasty using ‘autologous costal (rib) cartilage’ was planned.
2. Eyes
# The starting point of the eyelids was too thick and asymmetrical, creating an artificial look. Her eyelid opening strength was also weak.
# We planned to create a clear and defined impression by refining the lines through incisional ptosis correction.
* Surgical Plan
1. Nose
# 3rd Revision Rhinoplasty
: Bulbous nose & Nasolabial angle correction (Autologous costal cartilage)
2. Eyes
# Incisional ptosis correction










* Post-operative Findings
1. Nose
# Due to severe scar tissue, it was thoroughly removed, and the area was washed with a scar tissue inhibitor.
# The nasal septum was reconstructed. Autologous costal cartilage from the right 7th rib was harvested to create a sturdy pillar, and cartilage was grafted on top to elevate the nasal tip. Since the nose was not low to begin with, further elevation was challenging, but the sturdy pillar allowed for a sharp and sophisticated nasal tip.
# Although there were limits to the improvement due to her naturally thick nasal tip skin, the patient was very satisfied with the noticeable change compared to before the surgery.
2. Eyes
# Through incisional ptosis correction, we achieved clear and symmetrical lines!
